GM Early Years App - Your child's developmental assessments online

The health visiting service for children in Greater Manchester is changing. The developmental assessment forms for children aged 0 - 2.5 years old have moved online.

The assessment forms were previously completed on paper, but you can now access them via a web browser link or by downloading the GM Early Years App on your smartphone. This will help to ensure children are developing as expected and are provided with additional support when needed.

For parents and carers, it will:

  • Make it easy to access and complete forms on your mobile, tablet or computer - no longer will you need to worry about losing or forgetting the paper forms.
  • Provide videos and other guidance to support your child's development.
  • Give you online access to your child's development records whenever you need them.
  • Enable your health visitor to share documents with you through the App

The Early Years project is a partnership between the NHS in Greater Manchester, the Greater Manchester Combined Authority (GMCA) and Local Authorities across the city-region.

What is the Early Years App?

How do I access the GM Early Years App?

A member of your health visiting team will get in touch with you to confirm your email address and mobile phone number. You will then receive an email and text message with a link to create your account, along with a help guide.

In order to access the Early Years application you will need to create an NHS Login (if you don't have one already). You will need to provide your name, date of birth, NHS number and postcode. You will receive an email from the NHS once they have verified your details and you can then use the same NHS Login credentials to log into the Early Years Application. Once in the Early Years application you can start completing your child's ASQ assessment forms.

Within the app you will also have the option to increase your level of access to view additional functionality, such as your child's historical assessment forms and any documents shared by your child's Health Visitor. To do this you will need to submit photo ID or provide registration details you may have been given when registering with GP services, as well as recording a short 5 second video to verify your identity.
